Dorset Supported Self-Management Service

Keeping healthy and feeling well often requires us to feel confident in ourselves and connected to our local community. However, sometimes it can be difficult to know what to do, what’s available and how to connect to it.

The supported self-management service supports people with long-term health conditions to feel more confident in their ability to manage in everyday life.

 

Can I access this service?

If you are over the age of 18, and live in Dorset, then yes!

Do I have to pay for the service?

No. This service is funded by NHS Dorset and carried out by Help & Care. All sessions are completely free of charge for users.

How does it work?

Our commitment is to work with you as an equal partner to help you look forward, starting the conversation with what matters to you. Wherever you are on your journey we have tailored support to suit you.

 We take a person-centred approach to supporting people, as we know that one size does not fit all. As part of this. our dedicated, compassionate teams are trained in coaching techniques, as well as having links to other organisations within our community to ensure you get the support you need.

We offer one to one health and wellbeing coaching, as well as group coaching. Group coaching takes the form of our HOPE programme, which you can find out more about here.

Is it online?

We understand that while many services operate purely online, we want to be available for people wherever it suits them. Therefore, this service can be accessed by:
